By decision of the Board of Directors of Central Securities Depository JSC (the Central Securities Depository), at its meeting held on 21 May 2026, additions and change No. 1 to the Forms of operational documents used by Central Securities Depository JSC in its registrar and depository activities (Forms of operational documents) have been approved.

The amendments provide for the following changes:

1)      addition to the Report Form on execution/non-execution of the order (Form 54) with the field “Information about the unique code”, which is stipulated to be filled in response to the Order to assign a unique code to the depositor's client;  

2)  addition to the Application form for acceptance of a financial instrument for depository services (Form 59) with the fields “Name of the issuer” and “Type of financial instrument”;

3)  introduction of a new form containing information on issuing the securities of the local executive body (Form 71), which will be submitted by local executive bodies.

Additions and change No. 1 to the Forms of operational documents will be effective from May 22, 2026.
